ESCAPE 2018 - INSTRUCTOR AGREEMENT FORM
INSTRUCTIONS:
  • To assist in the organization and scheduling of the training tracks, all instructors must complete the Instructor Agreement  Form,
    including a brief biography and course description, for EACH class they are teaching.   Again, if you are teaching more than 1 class, you must submit the information separately.
  • Submit your Instructor Agreement Form(s) NO LATER than MARCH 1, 2018.
  • Completed forms will be forwarded to your track coordinator, Program Lead, and ESCAPE Chair.
Registration Fee Waivers Policy
  • For Teaching 4 + Hours, we waive your registration fee.
  • For Teaching 2 + Hours (but fewer than 4), we will waive 50% of your registration fee.
  • For Teaching 1 Hour, you will need to pay full registration.
  • Teaching assistants/helpers pay full registration.
  • Regardless of the policy, you are welcome to pay the registration fee if you wish to support ESCAPE.
